How do I submit my immunization information?+
Please follow the 2-step process detailed on the Penn State Immunization Compliance page.
Step 1: PRINT and TAKE a copy of the Penn State Immunization Verification Form to your health care provider for completion.
Step 2: Log in to myUHS using your Penn State access credentials, and upload images of the completed Penn State Immunization Verification Form and, if applicable, any positive antibody titer blood test results.
What immunizations are required?+
Penn State requires measles, mumps and rubella for all incoming undergraduate and graduate students and meningococcal conjugate vaccine (serogroup ACWY) for all students living in University housing. All other vaccines are strongly recommended but not required.
What should I do if I have problems uploading my immunization records through the secure myUHS portal?+
First, make sure you are trying to upload an image file. We accept PNG, JPG, JPEG, and GIF image files that are less the 4MB in size. You may also want to clear your browser cache and cookies or try using a different browser
If you are not successful after implementing these fixes, contact us via email and include the following information: details of the error message you are receiving, a screenshot (if possible), the name of the internet browser you are using (i.e. Explorer, Safari, Chrome, Firefox), and type of device (i.e. desktop computer, iPad, iMac, mobile phone). Also, please include your name and PSU ID#.
I can't find my immunization records. Do I need to have the immunizations again?+
Your health care provider can determine if you need the immunizations or if blood tests should be performed to determine immunity. He or she can then enter this information on the required Penn State Immunization Verification Form. You can have these tests done at University Health Services, your campus health center, or a local provider's office at your own expense if they are not covered by your health insurance.
How much do immunizations cost?+
Students with Penn State Student Health Insurance are fully covered for required immunizations. If you do not have Penn State Student Health Insurance, please check with your health insurance carrier to see what immunizations are covered and how you can be reimbursed if you receive the immunizations at University Health Services, your campus health center, or a local provider's office.
Can I submit a copy of my immunization records instead of the Penn State Immunization Verification Form?+
No. The Penn State Immunization Verification Form is required to be completed, signed, dated, and stamped by a health care professional.
I do not live in University housing. Am I still required to receive a meningococcal conjugate vaccine or submit a waiver?+
No, only students living in University housing are required to have a meningococcal vaccine or submit a waiver; however, it is strongly recommended for all students living in dorms, apartments and communal housing.
I have copies of my immunization records but I do not currently have a health care provider to complete the form, what should I do?+
University Park students can schedule a form review appointment at University Health Services by calling 814-863-0774.
***You must bring your original childhood immunization records or a copy of your childhood immunization records from your health care provider to your appointment.***
Commonwealth Campus students can contact the campus nurse or health center at their campus (if available) for additional information. A health care provider may be available on campus to complete your form, or you may need to contact a health care provider in your local community.
What if I am unable to receive the MMR vaccine due to a medical reason or condition?+
Print a copy of the MMR Waiver Request Form and have it completed and signed by your health care provider. You may fax the form to 814-865-9309 or mail the form to:

Do parents have access to myUHS to upload student vaccination information?+
Access to University Health Services secure portal (myUHS), is for students only. Students will need to log in using their Penn State access credentials (User ID and password). Sharing student access information with anyone--including a parent--is against Penn State security policies.
My immunization records are in a foreign language. Will the University accept these?+
No. All documentation submitted for immunization compliance must be in English. Contact your health care provider for a translated copy.
Are immunization requirements applicable to law and medical students?+
Penn State immunization requirements are applicable only to law and medical students studying at the University Park campus. Students at the Dickinson School of Law and Penn State Health Milton S. Hershey Medical Center are not required to submit immunization information through myUHS.
Do immunization requirements apply to non-degree seeking students?+
Non-degree seeking students are not required to submit immunization information; however, non-degree seeking students living in University housing must provide proof of the meningococcal conjugate vaccine or submit a Meningococcal Vaccine Waiver (students aged 22 and older only).
